In order to become a Weissman Designs for Dance customer, you will need to provide the following for proof of your business affiliation:
  1. Yellow Pages Ad
  2. Pre-printed bank deposit slip
  3. Pre-printed voided business check
  4. Photocopy of your dance school business license
  5. Brochure or program with your name listed as an       instructor, manager or owner
  6. Website address with your name listed as an instructor,       mananger or owner
